The MATA Honors and Awards Committee would like to thank you for Hall of Fame nomination.  As the sponsor, you are responsible for completing and submitting the below online nomination form in its entirety.  Incomplete information could delay the nomination process.  Thank you again for your support of the award candidate. 

All candidate information must be received by February 1st annually.

Letters of Recommendation: It is required of the sponsor to solicit two (2) letters of recommendation printed on letterhead stationary.  One of these must be from a licensed physician. Please do not send more than three letters of recommendation.  Letters of recommendation are based on quality not quantity. All letters must be signed and delivered with all other paperwork.  Please keep this nomination confidential!

Resume and Curriculum Vita: A current resume and/or curriculum vita (preferred) is to be included with your nomination form. This is to supplement the information that you provide in the nomination form.

Evaluation Process: The review process takes several weeks. The MATA Honors and Award Committee will evaluate each nomination, complete an evaluation form, and score each nomination reviewed.

Notification: All sponsors and candidates will be notified of the selection of award recipients in March.

Awards Presentation: Each award recipient will receive an award designating the level of award.  The award presentation will take place at the MATA Annual Meeting and Clinical Symposium.
